3. Client Service Agreement & Terms of Participation
1. Overview of Services
Meals With A Friend (MWAF) provides the following to clients:
Delivery of home-cooked or prepared meals by volunteers
Friendly visits offering conversation, companionship, and light check-ins
Flexible companionship activities based on client interests (e.g., playing cards, sharing stories, puzzles, watching sports, or walking)
MWAF does not provide medical care, personal care, or medication management.
2. Eligibility
MWAF welcomes clients of all ages and abilities who:
May be experiencing loneliness, disability, or difficulty accessing meals or companionship
Live within the current MWAF service area (Marinette, Peshtigo, Menominee, and surrounding areas)
3. Expectations of Clients
Clients agree to:
Communicate dietary needs or safety concerns clearly
Be respectful and courteous to volunteers
Notify MWAF if they need to cancel a scheduled visit
Understand that volunteers may stay 30–90 minutes, depending on availability and the client’s needs
4. Cancellations / Missed Visits
If a volunteer arrives and no one is available within 5–10 minutes, the visit will be marked as complete, and a standard visit fee may still apply unless prior notice was given.
5. Fees & Donations
MWAF operates on a hybrid model with:
A small participation fee per visit (varies by location and meal type)
Donation-based support for those who qualify.
No one is turned away for inability to pay, but consistent communication is expected.
6. Consent and Liability Waiver
By signing this form, you acknowledge:
You understand the nature of the services provided
You release MWAF and its volunteers from liability for risks associated with home visits and meals
You grant permission for MWAF to use your de-identified feedback to improve services
7. Description of Services
Meals With A Friend (MWAF) provides the following services:
Delivery of home-cooked, freshly prepared, purchased, or restaurant-provided meals to clients’ homes
Friendly visits, conversation, and light social interaction from volunteers
Light observation and informal emotional support (non-medical)
Client Expectations:
Treat volunteers and staff with courtesy and respect
Communicate any schedule changes promptly
Inform MWAF of any relevant health or home access information
Be available at the scheduled delivery/visit time
8. Medical Disclaimer
Meals With A Friend does NOT provide medical care, emergency medical assistance, nursing services, or personal care.
Our volunteers are not trained medical professionals and cannot assist with medication management, physical care, or medical emergencies.In case of a medical emergency, the volunteer will call 911 and then MWAF staff.
9. Cancellation and Rescheduling Policy
If you need to cancel or reschedule a meal delivery or friendly visit:
Please notify MWAF at least 24 hours in advance when possible.
Repeated last-minute cancellations may result in a review of continued participation.
If no one is available at the time of the scheduled visit, the volunteer will wait 5–10 minutes. If no one arrives, the volunteer will leave, and the missed visit will still be recorded.
10. Fees and Donations
Meals With A Friend currently operates through a combination of donations and sponsorships.
There is a required fee at this time.
Donations are gratefully accepted to help sustain and grow the program.
Information about how to donate (online, by mail, or in person) is available on our website and through staff or volunteers.
Meals With A Friend reserves the right to update donation policies as the program evolves.
11. Consent to Services
By signing below, you consent to:
Receiving meal deliveries from MWAF volunteers
Receiving friendly visits from MWAF volunteers
Allowing authorized MWAF volunteers access to your home (within your instructions)
Allowing MWAF to retain your information securely for service purposes
You also acknowledge that participation is voluntary and that either party may terminate participation at any time with notice.
