Volunteer Waiver and Release of Liability

VOLUNTEER WAIVER AND RELEASE OF LIABILITY
This Volunteer Waiver and Release of Liability ("Agreement") is made and entered into as of the date signed below by and between Meals With A Friend, LLC ("Company") and the undersigned individual ("Volunteer").
1. VOLUNTARY PARTICIPATION: The Volunteer acknowledges that participation in activities related to the company is voluntary. The volunteer understands and agrees that the services provided are for charitable and community-based purposes, and compensation will be provided based on time spent, involvement, and feedback.
2. ASSUMPTION OF RISK: The volunteer understands that participation may involve risks, including but not limited to physical injury, illness, accidents, or property damage. The volunteer assumes all risks associated with participation and agrees to take full responsibility for personal safety.
3. RELEASE AND WAIVER OF LIABILITY: To the fullest extent permitted by law, Volunteer releases, waives, and discharges the company, its owners, directors, officers, employees, agents, and affiliates from any and all claims, liabilities, demands, damages, costs, expenses, or causes of action, known or unknown, arising out of participation in volunteer activities.
4. INDEMNIFICATION: The volunteer agrees to indemnify and hold harmless Meals With A Friend from any claims, losses, liabilities, damages, costs, or expenses (including attorney fees) arising from the volunteer’s actions, negligence, or misconduct.
5. MEDICAL TREATMENT: The volunteer consents to emergency medical care if needed during participation and acknowledges that the company does not provide medical insurance coverage.
6. NO EMPLOYMENT RELATIONSHIP: The volunteer acknowledges that they are not an employee, contractor, or agent of the company and will not receive compensation, benefits, or workers’ compensation coverage.
7. PHOTO AND MEDIA RELEASE: The volunteer grants the company the right to use photographs, videos, or recordings taken during participation for promotional and marketing purposes without compensation.
8. GOVERNING LAW: This Agreement shall be governed by the laws of the state of Wisconsin & Michigan, where the company operates. Any dispute shall be heard by the Circuit Court sitting in Marinette & Menominee Counties.
9. SEVERABILITY: If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
10. CRIMINAL HISTORY DISCLOSURE: The Volunteer acknowledges and agrees to disclose any past criminal history or pending legal violations. The Company reserves the right to evaluate such disclosures to determine eligibility for participation in volunteer activities.
11. ACKNOWLEDGMENT AND SIGNATURE: By signing below, the volunteer acknowledges having read and understood this Agreement and voluntarily agrees to its terms.
